Student-Athlete forms

Marietta College student-athletes are annually required to fill out and return an insurance form and sports medicine athlete history form before they can participate in any athletic-related activity.

In addition, first-time student-athletes are also required to fill out the physical form.

The following links have all of the appropriate forms, depending on your status as either a new student-athlete or returning student-athlete.
