Webadmin Work Request Form
When submitting a request, please note the following items:
—each department should designate one contact person who is responsible for submitting requests and maintaining the site,
—each request should be as detailed as possible and indicate if this is a time-sensitive matter,
—if you are requesting changes to a number of Web pages, please paste each URL into the Details of Request field,
—each contact person will be asked to provide login information to departmental pages at a future date.
Do not include the login and password information with the initial work request.
